Homeless Prevention Rental Assistance Program
View a list of the documents required to be submitted for every member of the applicant’s household for the HP-RAP Program.
Required Documents
An application will not be considered complete, and assistance will not be processed, unless all applicable documents are provided for everyone in the household, and within the time/deadline indicated.
- Copy of one of the following: government issued photo I.D.s: Driver’s license, State of Florida ID or passport) for everyone age 18 or older
- Copies of birth certificates, hospital footprints for newborn 30 days or younger, school ID or passport for each person under age 18 in the household
- Copies of the Social Security letter for everyone in the household reported as disabled or receiving income from Social Security
- Copies of bank statements (most recent last two months) for all adults in household
- Paycheck stubs before COVID-19 AND if currently employed, current pay stubs with reduced hours or pay
- Letter from employer stating your job loss, or reduced work schedule and hours, or completed Income Verification Form (this form can be used if you do not have a letter from your employer)
- Unemployment Claims Benefit Letter or proof that you filed for unemployment with the State of Florida
- No Income Affidavit Form (if any household member of age 18 or older is claiming zero income, this form must be completed and submitted)
If you or a member of your household are self-employed, you must provide evidence for loss of income including these three items:
- 2019 signed tax return
- Signed and current profit and loss statement (January 2020 through current)
- Unemployment claim benefits letter (Pandemic Unemployment Assistance) from the Department of Economic Opportunity or unemployment submitted application
Please note that a Processor may also request additional information including but not limited to bank statements or copies of documentation to demonstrate your loss or inability to pay your rent.
If requesting assistance for rent
- Copy of current, executed lease agreement (must be signed by landlord/tenant)
- Rent roll or general ledger provided by landlord showing rent is past due
- Copies of the following two documents from your landlord:
- IRS Form W-9 (we will send you blank form)
- ACH Direct Deposit form (we will send you blank form)
If requesting assistance for utilities (electric, water, gas)
- Copy of most recent utility bill showing past due amount

- Adobe Acrobat PDF Files (.pdf)
- Image Files (.jpg, .jpeg, .bmp, .png, .tiff)
- The maximum size allowed per upload is 25MB

Low-Income Eligibility
This program is intended to serve individuals with low income. To determine if you meet income eligibility, look at the chart below and see that maximum income permitted for your household to qualify for this assistance. The “Persons Living in Household” includes the head of household (applicant).
