Camillus Health receives Level 3 patient centered medical home recognition

Camillus Health has been recognized on behalf of the National Committee for Quality Assurance as a Level 3 Patient Centered Medical Home for providing top quality care using systematic, patient-centered and coordinated care management processes. This prestigious level of recognition has only been awarded to 20 health centers in the state of Florida.

Patient Centered Medical Home is an approach that provides comprehensive primary care that facilitates partnerships between individual patients, their doctors and, when appropriate, the patients' families. The goal is to allow better access to health care, increase satisfaction with care and improve overall health.

They have continued to provide the best quality service and improve the health of persons experiencing homelessness through best-practice preventive measures.”

Patient Centered Medical Home evaluates 27 elements within six overall standards, including access and continuity, identify and manage patient populations, plan and managed care, self-care, track and coordinate care, and performance measurement and quality improvement.
